ACL surgery rehabilitation in Bangkok
Get back to sport with a structured plan, expert guidance, and a team that tests before they clear you
ACL reconstruction is one of the most common surgeries among athletes and active individuals and one of the most mismanaged in recovery. The surgery fixes the ligament. Rehabilitation is what determines whether you return to sport safely, confidently, and without re-injury. At Form Recovery & Wellness in Bangkok, we guide you through the full 9–12 month journey from post-operative week one through return-to-sport clearance, using criteria-based testing at every milestone.
Most patients we see after ACL surgery are not recovering slowly because of the graft, they are recovering slowly because they don’t have a structured plan, their quad hasn’t been properly activated, or they were cleared on a calendar date rather than functional criteria. We fix that.
Our physiotherapy team combines, when needed, hands-on Physiotherapy rehabilitation with Traditional Chinese Medicine (TCM) and Applied Thai Traditional (ATM) medicine support to manage pain, reduce swelling, rebuild muscle strength, increase range of motion and mobility, and get you back to doing what you love — whether that’s football, padel, trail running, or simply being active again without fear.
We understand the frustration — and the fear — that can come with ACL surgery recovery. For many patients, the hardest part isn’t the surgery itself. It’s the long months that follow, often without a clear timeline, without someone testing whether your knee is actually ready, and without honest answers about re-rupture risk.
You may have come to us because one of these situations sounds familiar:
- It has been 3 months since your surgery and you still cannot jog — and your surgeon has not given you a clear explanation of why or what to do next
- You passed the "6-month mark" but your knee does not feel ready to go back to sport, and you are not sure who to ask
- Your quad is visibly weaker on the operated leg and nobody has formally tested your limb symmetry
- You had ACL surgery abroad — in the UK, Australia, the US, or elsewhere in Asia — and returned to Bangkok without a structured rehabilitation protocol
- You are afraid of re-rupturing, so you have been avoiding all physical activity — but that avoidance is actually slowing your recovery
- Your knee feels mechanically stable but your confidence does not match — and you know the psychological side of ACL recovery is real but nobody has acknowledged it
- You were discharged after a few sessions of post-op physio and told to "keep doing your exercises" — with no follow-up testing or progressive loading plan

Our approach to ACL rehabilitation follows three principles:
Listen First
Before we prescribe anything, we want to understand your surgery — what graft was used, what your surgeon's protocol says, where you are in your recovery, and what your goal actually is. Getting back to competitive football is a different program from getting back to weekend hiking. We build your plan around your goal.
Guide you through every phase
ACL rehabilitation has six distinct phases, each with specific goals and measurable milestones. We don't advance you to the next phase on a calendar date. We advance you when your body is ready — and we test that objectively. Quad strength symmetry, hop tests, range of motion, agility benchmarks: these are the checkpoints that determine your progression.
Get you back properly.
Return to sport is not the finish line. Return to sport with strength symmetry above 90%, passing hop test criteria, and psychological readiness is the finish line. We stay with you until you cross it.
Our team finds the right treatment combination of Physiotherapists, TCM Practitioners, and Applied Thai Traditional Medicine Practitioners working together on your case. If your quad is inhibited, dry needling can help. If post-surgical inflammation is slowing your progress, acupuncture supports recovery without additional medication. This multidisciplinary approach means we can respond to what your knee needs — not just follow a generic post-op sheet.
What your rehab program at Form Recovery & Wellness includes
Your first session is a full assessment, not a treatment session. We want to understand exactly where you are before we plan where you’re going.
- Review of your surgical report, graft type, and surgeon's rehabilitation protocol
- Type and mechanism of injury (how you sustained the ACL tear), and which specific symptoms or limitations have not yet improved
- Current range of motion assessment (flexion and extension)
- Swelling and effusion assessment
- Quad activation test and muscle strength evaluation (both legs)
- Special tests for knee ligament integrity and joint stability
- Functional movement screening appropriate for your phase
- Pain assessment and pain pattern analysis
- Review of your pre-surgery activity level and return-to-sport goal
- Discussion of rehabilitation timeline, phase milestones, and what to expect
- Design of your personalised ACL rehabilitation program
- Hands-on physiotherapy: joint mobilisation, manual therapy, soft tissue work
- Targeted exercise progression: from quad sets and straight-leg raises in Phase 1 to single-leg squats, plyometrics, and sport-specific drills in later phases
- Neuromuscular and proprioception training: balance boards, perturbation training, reactive movement drills
- Swelling management: ice, elevation, compression, muscle pumping exercises, and gentle joint movement
- Progress testing: we re-measure range of motion, strength, and function at each milestone
- Adjunct treatments where appropriate: dry needling, acupuncture, red light therapy, or aqua therapy
- Education: you leave every session knowing what your numbers are, what the next target is, and exactly what to do between sessions
What does the recovery timeline look like after ACL surgery?
ACL reconstruction has the longest rehabilitation timeline of any common orthopaedic procedure. Nine to twelve months is the standard, and that timeframe exists for a biological reason: the graft goes through a process called ligamentisation — gradually transforming from transplanted tissue into functional ligament — and this process takes most of that first year. Returning to pivot and contact sports before 9 months significantly increases re-rupture risk. This is not a conservative estimate — it is supported by strong clinical evidence.
Below is the standard recovery timeline. Actual progression depends on your graft type, your surgeon’s protocol, your starting fitness level, and how your body responds to each phase.
Immediate Post Op
Frequency: 3–5x per week
Key Goals: Pain and swelling control, full extension (0°), quad activation, prevent muscle atrophy
ROM Target: 0° to as far as tolerated
The first week is not about rehabilitation in the traditional sense. It is about protecting the graft, controlling pain and swelling, and preventing the quad shutdown that almost universally follows ACL surgery. Achieving full extension (0°) in the first week is critical — extension loss that becomes established is significantly harder to recover. Ice, elevation, compression, and gentle quad activation exercises are the priority.
Move on when:Full extension is achieved, swelling is reducing, quad is activating, and you can flex to 90°.
Early Strengthening
Frequency: 2–3x per week
Key Goals: Full ROM 0–120°+, progressive weight-bearing, quad and hamstring activation, proprioception begins
ROM Target: 0–120°
Weight-bearing increases progressively. The goal is to reach full range of motion (0–120° or beyond), begin building quad and hamstring strength bilaterally, and introduce basic proprioception and balance training. Open-chain exercises (leg extensions) are typically restricted in early phases depending on graft type — your physiotherapist will follow your surgeon's specific protocol.
Move on when:Full ROM is restored, you can walk without a limp, and early strength benchmarks are met.
Progressive Strengthening
Frequency: 2x per week
Key Goals: Single-leg exercises, jogging preparation, sport-specific movement patterns
ROM Target: Full ROM
Single-leg exercises begin. Leg press, step-ups, single-leg squats (bodyweight), and Romanian deadlifts. Bilateral exercises are also progressed — bilateral squats and deadlifts, deepest squat as tolerated, and lunges. Proprioception training becomes more challenging — unstable surfaces, single-leg balance with perturbation. Light cycling is usually introduced. The operated quad is being brought back toward symmetry with the opposite leg, which is the foundation of safe return to running.
Move on when:Single-leg strength and control meet phase criteria, no effusion after exercise, jogging preparation exercises are well-tolerated.
Running & Agility
Frequency: 1–2x per week
Key Goals: Running program, cutting and pivoting drills, sport-specific conditioning
ROM Target: Full ROM
Straight-line jogging begins on a flat surface. This is a milestone most patients look forward to — and one that should be introduced progressively, not suddenly. A structured return-to-running program typically starts with walk-jog intervals and builds over 4–6 weeks. Cutting, pivoting, and change-of-direction drills are introduced late in this phase, after the knee demonstrates stability under load.
Move on when:30-minute continuous jog achieved, no pain or swelling, cutting drills are performed with confidence and control.
Return to Sport Prep
Frequency: 1x per week + independent training
Key Goals: Strength symmetry >90%, hop tests, reactive agility, sport-specific drills
ROM Target: Full ROM
The most psychologically demanding phase. Strength testing (typically isokinetic or functional limb symmetry index) is performed. The goal is >90% symmetry between the operated and non-operated leg. Hop tests (single, triple, crossover, 6-metre timed) are administered. Sport-specific drills — heading, throwing, catching, reactive change of direction — are reintroduced. Psychological readiness is assessed using validated tools such as the ACL-RSI (Return to Sport after Injury) questionnaire.
Move on when:Strength symmetry >90%, all four hop tests passed, psychological readiness confirmed, no pain or swelling with full sport-specific training loads.
Clearance & Maintenance
Frequency: As needed, milestone-based
Key Goals: Return-to-sport criteria met, psychological readiness, ongoing maintenance
ROM Target: Full ROM
Formal return-to-sport testing is completed. We work with your surgeon for final clearance. Maintenance strength and conditioning continues to reduce long-term re-injury risk. Many patients continue monthly check-ins through this phase to monitor the knee under full sport loads.
Move on when:All return-to-sport criteria are met and surgeon clearance is confirmed.

Following a knee injury, I went through acl surgery + meniscus repair. This facility help me with my rehab. I have a main physiotherapist that follows up my recovery process. They are very attentive, help me with my pain, gave me motivation to exercise even when it is tough. I have exercises to do at home + 2 sessions a week. They use well regconize scientific literature on recovery and that is what make me trust them at the first session. Now my trust is built on their detailed explanations, their constant care, their cheerful outlook and the results that we got together. Also they are easy to reach, very service oriented which is always nice. I hope I could had this type of service sooner because they help me with so much more than my knee (posture, back pain,...)Load more
How different treatments at Form Recovery & Wellness support ACL surgery recovery
Most clinics offer physiotherapy alone after ACL reconstruction. At Form Recovery & Wellness, our physiotherapists work alongside TCM practitioners and Thai traditional medicine therapists — which means we have more tools to address the specific challenges that come up in ACL rehabilitation, from quad inhibition in week one to donor-site restrictions in month three.
Physiotherapy
- Restores full ROM, rebuilds quad and hamstring strength, develops proprioception, administers return-to-sport testing
- Best for core rehab — all patients
- Phases 1-6
Aqua Therapy
- Joint-friendly strengthening in water — builds muscle with significantly reduced load through the graft site
- Best for patients with high pain, swelling, or fear of load bearing; early strengthening phase
- Phases 2-4
Dry Needling
- Decreases pain and improves range of motion; releases quad inhibition (arthrogenic muscle inhibition) and relieves muscle tightness around the knee — both common post-ACLR
- Quad activation difficulty, poor muscle activation, trigger points, muscle guarding
- Phases 2-3
Acupuncture
- Reduces post-surgical pain and inflammation, supports nervous system recovery and sleep quality
- Pain management without additional medication
- Phases 2-4
Red Light Therapy
- Promotes tissue healing at graft integration sites, reduces local inflammation and wound healing time
- Wound healing, early post-surgical recovery
- Phases 1-3
Your physiotherapist will recommend a combination based on your graft type, your current phase, and what your knee is telling us at each assessment. You are never prescribed treatments you don’t need.
What can you expect from structured rehabilitation?
Without structured rehabilitation after ACL surgery, the risks are significant. Quad inhibition — the neurological shutdown of the quadriceps that follows knee trauma and surgery — does not resolve on its own with time and casual exercise. Without systematic reactivation and progressive loading, patients are left with a weaker leg, poorer proprioception, and a substantially higher risk of re-rupture or secondary injury when they eventually return to activity.
With a structured, criteria-based ACL rehabilitation program at Form Recovery & Wellness, you can reasonably expect:
- Full range of motion restored — 0° extension and 130°+ flexion — within the expected timeframe
- Progressive return of quad and hamstring strength, tested objectively and tracked throughout recovery
- Swelling that responds to treatment and does not limit your progression between phases
- Proprioception and neuromuscular control that protects the graft during dynamic movement
- A safe, progressive return to running — typically at 3–4 months — built on objective readiness criteria rather than a calendar date
- Strength symmetry above 90% between both legs before return-to-sport clearance
- Reduced re-rupture risk through criteria-based return-to-sport testing (passing the hop tests, strength symmetry, and psychological readiness assessment before going back to competition)
- Confidence — both physical and psychological — when you return to your sport or activity
- A clear, honest plan at every stage: what today's goal is, what the next milestone is, and how close you are to getting back to what you love
When should you seek professional rehabilitation?
You should seek structured rehabilitation immediately after ACL surgery — ideally starting within the first week. But if you are already further into your recovery and experiencing any of the following, come and see us now rather than waiting:
- Your quad strength is less than 70% of the opposite leg at the 3-month mark
- You are unable to fully straighten your knee (0°) or bend past 120° by week 6
- You have significant swelling that is worsening rather than improving after exercise
- You are feeling sharp pain in the graft area during exercises or daily activities
- You have not been formally tested for return-to-sport readiness and are approaching the 9-month mark
- Your recovery has stalled and nobody has explained why or given you a path forward
These are not signs of failure. They are signs that you need better support — and that is exactly what we are here to provide.

One of the most common questions after ACL surgery is what activities are safe and when. Below are general evidence-based guidelines. These timelines assume you are recovering well and following a structured rehabilitation program.
General Timeline: 1-2 Weeks
Notes: Keep the knee elevated and iced as needed; take regular movement breaks
General Timeline: 2–4 weeks
Notes: Confirm with your surgeon based on pain and swelling
General Timeline: 4–6 weeks
Notes: Must have full control and confidence for emergency braking
General Timeline: 2 weeks
Notes: Move the ankle and knee regularly; compression socks recommended
General Timeline: 4–6 weeks
Notes: DVT risk is elevated post-surgery; follow your surgeon’s guidance
General Timeline: 1–4 weeks
Notes: When you can walk with a normal gait pattern and no limp
General Timeline: 4–6 weeks
Notes: Low resistance, full extension maintained
General Timeline: 3–4 months
Notes: Criteria-based — only when quad symmetry and ROM allow
General Timeline: 6 months+
Notes: After passing agility and strength symmetry tests
General Timeline: 6–9 months
Notes: Criteria-based return, not calendar-based
General Timeline: 9–12 months
Notes: After passing full return-to-sport criteria and receiving surgeon clearance
Every patient is different. Always follow your surgeon’s specific post-operative instructions. The above are general guidelines only.
Frequently Asked Questions
Nine to twelve months is the standard timeline for ACL reconstruction rehabilitation, particularly for athletes returning to pivot or contact sports. This is significantly longer than most patients expect, and the reason is biological: the graft undergoes a process called ligamentisation — transforming into functional ligament tissue — that takes most of the first year. Returning before 9 months without passing return-to-sport criteria substantially increases re-rupture risk. Timeline should always be criteria-based, not calendar-based.
Within the first week is ideal. The goals in weeks one and two are not strengthening — they are achieving full extension (0°), controlling swelling, and reactivating the quad. Quad inhibition (arthrogenic muscle inhibition) sets in quickly after knee surgery, and the earlier intervention begins, the easier it is to overcome. Starting rehabilitation early also means you start the whole timeline earlier — which matters when you are working toward a 9–12 month goal.
Yes — quad inhibition is almost universal following ACL reconstruction. It is caused by arthrogenic muscle inhibition: a neurological reflex response to joint effusion and trauma that essentially switches the quadriceps off. It does not resolve on its own with time. Specific quad activation techniques — including neuromuscular electrical stimulation, dry needling, biofeedback training, and targeted open-chain exercises — are used to overcome it. Formal limb symmetry testing at key milestones tracks your progress objectively.
Absolutely. We regularly work with patients who had ACL reconstruction in the UK, Australia, the United States, Singapore, or elsewhere in Asia and returned to Bangkok for their rehabilitation. Bring your surgical report, discharge summary, and your surgeon’s rehabilitation protocol — or we can work from standard evidence-based guidelines if that documentation is not available. Where possible, we coordinate directly with your surgeon’s protocol to ensure continuity of care.
Return to contact and pivot sports requires passing a full suite of return-to-sport criteria: strength symmetry above 90% (limb symmetry index), all four hop tests (single, triple, crossover, 6-metre timed), functional movement screening, and psychological readiness assessed using a validated questionnaire such as the ACL-RSI. Time alone is not a criterion. The minimum safe timeframe for pivot and contact sport is 9 months, with most athletes reaching clearance between 9 and 12 months when following a structured program.
Re-rupture risk is significantly influenced by the timing and quality of return-to-sport. Research by Grindem et al. (BJSM, 2016) found re-rupture rates of 15–25% in young athletes who returned before 9 months, compared to approximately 6% in those who waited beyond 9 months and passed formal return-to-sport criteria. Each additional month of delay beyond 9 months reduces re-rupture risk by approximately 51% in young athletes. This is why we take criteria-based return-to-sport seriously — not to be conservative, but because the evidence strongly supports it.
