Hip Replacement Rehabilitation in Bangkok
Hip replacement rehabilitation typically takes 3–6 months to reach full functional recovery, with the most critical progress happening in the first 6–12 weeks. Evidence consistently shows that patients who begin structured physiotherapy early — ideally within 24–48 hours of surgery — achieve significantly better strength, mobility, and independence outcomes at 12 months compared to those who delay or rely on self-guided recovery.
At Form Recovery & Wellness in Bangkok, we find the best combination of Physiotherapy, Traditional Chinese Medicine, and Applied Thai Traditional Medicine to guide you through every phase of recovery after total hip arthroplasty (THA) — whether your surgery was performed in Bangkok or abroad. This guide covers your recovery timeline, hip precautions explained in plain language, the treatments that support each phase, and how structured rehab gives you the best long-term outcome from your new joint.
Hip replacement surgery replaces a worn or damaged joint — but the surgery itself is only the beginning. Getting back to the life you want means rebuilding the strength around the new joint, re-learning how to move safely, and gradually returning to the activities you’ve been missing. That process takes time, the right guidance, and the right team.
Some patients arrive at Form Recovery having done well in hospital but unsure what to do next. Others come to us weeks into recovery feeling stuck — still limping, still uncomfortable, wondering if this is as good as it gets. It’s not.
You might be dealing with one of these situations right now:
- You were discharged from hospital with a list of hip precautions — no bending past 90°, no crossing your legs — but no one clearly explained why, or how to actually manage those restrictions when getting into a car, sitting on the toilet, or putting on shoes.
- Your hip still feels stiff and your limp is still significant at 6 weeks, and you're not sure whether that's a normal part of recovery or a sign that something isn't progressing as it should.
- Your swelling is persisting much longer than you expected, and no one has addressed it — it's affecting your sleep and making you hesitant to move.
- You had your hip replacement in another country and returned to Bangkok without a structured outpatient rehabilitation plan in place.
- You're anxious about dislocating the hip — and that fear is quietly limiting your movement more than any physical restriction. You're holding back, and it's slowing your progress.
- You're doing reasonably well with your exercises, but your gluteal muscles still feel weak and you've developed compensatory pain in your lower back or opposite knee from the altered way you've been walking.
- You're older and living alone or with limited support — you need rehab that builds not just physical strength, but the confidence and independence to manage safely at home.
If any of these sound familiar, you’re not alone. Hip replacement rehabilitation is a structured program we run regularly at Form Recovery & Wellness, and we’ve helped many patients recover fully — regardless of when they started or what their starting point was.
The fact that you’re looking for the right guidance is already a good sign.

Listen First
We listen. We understand. And we assess your hip's current mobility, strength, and surgical factors — including your specific precautions and approach — in order to create a personalised rehabilitation plan for you.
Guide you through every phase
We guide you through your personalised rehab program — progressively building strength, restoring mobility within safe limits, and managing complications like swelling, muscle guarding, or altered gait as they arise.
Get you back properly.
Get back to doing the things you love.
Our goal is to get you back to full function — safely, efficiently, and with as little disruption to your life as possible. We do that by involving our entire team. Every case is regularly reviewed by the Head of each Department to give you the highest chance of success. It is not uncommon for our team to recommend a multidisciplinary approach between Physiotherapy, Traditional Chinese Medicine, and Applied Thai Traditional Medicine. After your consultation, we will recommend the treatment method that best suits your needs — whether that’s a combination of therapies or a single method.
What your hip replacement rehab program includes
- Review of surgical report, operative note, and surgeon's post-operative protocol
- Confirmation of surgical approach (posterior, anterior, or lateral) and associated hip precautions
- Hip range of motion assessment (flexion, extension, abduction, rotation)
- Strength testing of key muscle groups — gluteus medius, gluteus maximus, hip flexors, quadriceps
- Limb circumference measurement at the thigh to track muscle mass and swelling over time
- Functional assessments: sit-to-stand, bed transfers, and step tests appropriate to your current level
- Gait analysis — assessment of limp pattern, weight-bearing, and compensatory movement
- Pain assessment and current medication review
- Goal setting based on your specific lifestyle and desired activities
- Manual therapy for swelling reduction and soft tissue recovery
- Joint mobilization to restore hip mobility within safe precaution limits
- Guided progressive therapeutic exercises targeting the gluteal group, hip flexors, and stabilizing muscles
- Therapeutic modalities as appropriate: ultrasound (US), electrical stimulation (ES), and laser therapy to support tissue healing and assist with pain control
- Gait training — progressing from walking aid to independent walking with correct mechanics
- Hip precaution education for daily activities: getting in and out of a car, using the toilet, dressing, and stair technique
- Education on home exercise progression and safe activity management
What does the recovery timeline look like after hip replacement?
Recovery from total hip replacement follows predictable phases, though your individual timeline will vary based on age, fitness level before surgery, surgical approach, and how well the surrounding muscles were preserved during the procedure. Here is what to expect across each stage.
Hospital Stay
Key Goals: Safe mobility in bed (basic bed transfers), early mobilisation with walker, pain and swelling management, blood clot prevention, learn hip precautions
Target: Walking with walker
Physiotherapy begins in hospital, often within 24–48 hours of surgery. The focus is on safe mobility in bed (basic bed transfers), pain and swelling management, blood clot prevention, and learning your hip precautions. You will begin early mobilisation with a walker under supervision.Move on when:You are safely mobile with a walking aid and cleared for discharge.
Early Recovery
Key Goals: Follow hip precautions strictly, progress from walker to cane, reduce swelling, restore basic hip mobility
Target: Walk with cane
This is the most precaution-critical phase. Hip flexion beyond 90°, crossing the legs, and internal rotation must be avoided — especially with a posterior surgical approach. The physiotherapist guides you from walker to cane, reduces swelling, and restores gentle hip mobility.Move on when:You are walking comfortably with a cane and swelling is controlled.
Progressive Strengthening
Key Goals: Walk without aids, rebuild gluteal and hip strength, stair confidence, light daily activities
Target: Walk unaided
Once precautions are lifted or relaxed by your surgeon, strengthening of the glutes, hip abductors, and hip flexors becomes the priority. Most patients walk without aids by weeks 8–10.Move on when:You are walking without aids, managing stairs, and beginning light activities.
Advanced Rehab
Key Goals: Full functional strength, return to work, low-impact recreational activities
Target: Full daily independence
Functional strength, return to work, and reintegration into daily life. Balance and proprioception training is introduced. Low-impact recreational activities resume based on your progress.Move on when:You have returned to daily activities with minimal compensatory movement.
Long-Term Recovery
Key Goals: Maximum strength, balance and proprioception training, return to recreation and desired activities
Target: All desired activities
Continued strength and endurance gains. Return to specific recreation — swimming, golf, cycling. Balance and fall prevention remain a focus, particularly for older patients.
How different treatments at Form Recovery & Wellness support hip replacement recovery
Most clinics offer physiotherapy alone. At Form Recovery & Wellness, we combine multiple treatment approaches depending on what your hip needs at each stage of recovery.
Physiotherapy
- Restores hip mobility within safe precautions, rebuilds glute and hip strength, corrects gait
- Best for core rehab — all patients
- Phases 1–5
Aqua Therapy
- Joint-friendly strengthening in water — buoyancy reduces load on the new hip while allowing full movement patterns
- High pain levels, early strengthening, limited weight-bearing tolerance
- Phases 2–4
Medical Lymphatic Drainage Massage
- Reduces post-surgical swelling and fluid retention, softens fibrosis forming around the new joint
- Persistent swelling, fluid retention
- Phases 1–3
Acupuncture
- Reduces post-surgical pain and inflammation, supports nervous system recovery
- Pain management without additional medication
- Phases 2-4
Hot Herbal Compress
- Promotes circulation and supports recovery after more demanding strengthening sessions
- Overall recovery and muscle soreness
- Phases 2–4
What can you expect from structured rehabilitation?
Without proper rehabilitation after hip replacement, the surrounding muscles — particularly the gluteus medius and maximus — can remain weak for months, leading to a persistent limp, altered movement patterns, and secondary pain in the lower back and opposite knee. Scar tissue can also restrict the hip’s range of motion, and without targeted management, swelling can become chronic and slow the entire recovery process.
With a structured rehab program at Form Recovery & Wellness, you can expect:
- Safe restoration of hip mobility within surgical precaution guidelines
- Progressive rebuilding of gluteal and hip strength to support the new joint
- Reduction and management of post-surgical swelling, particularly in the first 6 weeks
- Correction of compensatory movement patterns that develop during the guarded phase
- Confidence walking without aids — typically by weeks 6–10
- Reduced risk of falls — recovering hip stability, balance, and proprioception is a core part of the program, especially for older patients
- A clear roadmap so you always know where you are in your recovery and what comes next
- Return to the activities that matter to you — whether that's walking the dog, travelling, playing golf, or getting back to work
When should you seek professional rehabilitation?
- No physiotherapy plan in place: You have been discharged from hospital without structured outpatient rehabilitation arranged
- Still on a walking frame at 6 weeks: With no meaningful improvement in mobility or weight-bearing capacity
- Significant limp persisting beyond 8 weeks: A limp at this stage is not "just normal" — it means the hip muscles are not functioning adequately
- Hip still not flexing to 90° by week 8: Particularly relevant if you had a posterior approach and precautions have been lifted
- Persistent swelling: Worsening or not improving, especially if accompanied by warmth or redness around the wound
- Fear-driven immobility: If anxiety about dislocation is preventing you from moving and progressing
- Sudden severe hip pain: Especially with a "pop", "clunk", or inability to bear weight. This requires immediate medical attention as it may indicate hip dislocation
Why choose Form Recovery & Wellness?

It is measured in what you can do again — walking comfortably through the airport, getting back on the golf course, picking something up off the floor without thinking twice. We build your rehabilitation around that goal, and we do not stop until you reach it.
What makes us different:
- Goal and Results Oriented: Our goal is to help you achieve your goal (get back to doing something) as fast as possible.
- Multidisciplinary approach: We find the optimal combination of Physiotherapy + Traditional Chinese Medicine (TCM) + Applied Thai Traditional Medicine (ATM) to treat from multiple angles
- No painkillers, injections, or surgery unless absolutely necessary. We manage pain and recovery through hands-on therapy and evidence-based treatment modalities.
- Clear communication— Patients always know what the problem is, what today's treatment is, next steps, and how close they are to their goal.
- 100% Happiness Guarantee
- Listen, Understand and We Genuinely Care for our patients.

One of the most common questions patients ask after hip replacement is when they can get back to specific activities. The answer depends on your surgical approach, your surgeon’s protocol, and your individual progress. Use the table below as a general guide — always confirm with your surgeon before resuming any activity.
General Timeline: 4–6 weeks
Notes: Right hip surgery requires longer as it affects braking. Confirm timing with your surgeon and ensure you are off opioid medications before driving.
General Timeline: 4–6 weeks
Notes: Wear compression stockings. Request a bulkhead seat or window seat for extra space and leg extension. Move regularly during the flight.
General Timeline: 6–12 weeks
Notes: DVT risk remains elevated post-surgery. Discuss directly with your surgeon before booking.
General Timeline: 2–4 weeks
Notes: Use a raised chair to maintain hip at or above knee level. No hip flexion beyond 90°.
General Timeline: 3–4 months
Notes: Only once the surgical wound is fully healed and closed.
General Timeline: 3–6 months
Notes: Low-impact activities; build gradually with physiotherapy guidance.
General Timeline: 6–12 months
Notes: Some patients are advised against high-impact activities long-term to protect the prosthesis. Discuss with your surgeon.
Timelines are general guidelines only. Your surgeon’s specific instructions take precedence. Posterior approach patients follow stricter precautions than anterior approach patients, particularly in the first 6–12 weeks.
FAQs About Hip Replacement Rehabilitation
Most patients walk without aids within 4–6 weeks and return to full daily activities by 3–6 months. Full strength and confidence — particularly for more demanding activities like golf, swimming, or extended travel — may continue improving for up to 12 months. The timeline depends on age, pre-surgery fitness, surgical approach, and consistency of rehabilitation.
Hip precautions apply primarily to the posterior surgical approach — the most common technique — and include: no hip flexion beyond 90° (no deep bending forward), no crossing the legs (no adduction past midline), and no internal rotation of the hip. These restrictions typically last 6–12 weeks, though some surgeons now use a posterior approach with modified or relaxed precautions. Anterior approach surgery often involves fewer restrictions. Always follow your specific surgeon’s instructions.
Physiotherapy begins in hospital, often within 24–48 hours of surgery. Outpatient rehabilitation typically starts at 1–2 weeks after discharge, once your wound is stable and you are safely mobile at home. Starting early significantly improves long-term outcomes — do not wait until you feel “ready enough.”
Yes. At Form Recovery & Wellness, we regularly combine physiotherapy with aqua therapy for joint-friendly strengthening — particularly useful when weight-bearing is still limited — and medical lymphatic drainage massage to reduce persistent post-surgical swelling. Your treating physiotherapist will recommend the combination that makes sense for your stage of recovery.
Driving after hip replacement is typically possible at 4–6 weeks, depending on which hip was operated on and whether you are still taking opioid pain medications. Right hip surgery generally requires a longer wait because braking involves that side. You must be able to perform an emergency stop safely before returning to driving. Always confirm with your surgeon before getting behind the wheel.
Hip dislocation is a serious complication that requires immediate emergency medical attention. Symptoms include sudden, severe hip pain — often with a “pop” or “clunk” sensation — a visible change in leg position, and inability to move or bear weight on the leg. If you experience these symptoms, do not attempt to move the hip yourself. Call emergency services or go to the nearest hospital immediately.
Form Recovery & Wellness offers expert hip replacement rehabilitation at our clinics in Asoke (Sukhumvit 20), Phrom Phong (Sukhumvit 24), Thonglor (Sukhumvit 49), as well as at Four Seasons Hotel Bangkok. Clinics are open weekdays 9am–8pm and weekends 9am–6pm. Home visits are also available for patients with limited mobility. Book your consultation to get started.
This information is for educational purposes only and does not replace professional medical advice. Always follow your surgeon’s specific post-operative instructions, as protocols vary significantly depending on surgical approach (posterior, anterior, or lateral), implant type, and individual factors. Hip precaution guidance in particular must be confirmed with your operating surgeon. Please consult with our practitioners or your healthcare provider to determine if this treatment is right for your specific condition.

I strained a muscle in my lower back. I couldn't even bend over to tie my shoes for 7 days, so I decided to get some physical therapy and found Form Recovery. Really nice facilites, very professional, thorough examination before starting therapy with Sutee/Jeng. I received massage, ultra sound and lazer treatment. He also provided some at home movements to perform to assist with the recovery. I only had 3 sessions because I was leaving Bangkok. But I feel much better, and my 18 hour flight home should be a lot more comfortable. I highly recommend Form Recovery. StuartPosted on Google![]()

Excellent Experience at Form Recovery - Highly Recommended! I visited Form Recovery for a consultation after experiencing problems with my neck, left shoulder and upper back for the past two months. I reached out to them online with a brief summary of my issues and they arranged an appointment for me quickly and effortlessly. I was assigned to Khun Panicha (Lucky), who was extremely thorough and attentive throughout the session. She started with a proper assessment of my movements and symptoms, followed by a detailed explanation of what might be causing my discomfort. This was followed by therapy and a series of exercises to help with my recovery and prevent the problem from recurring. Khun Lucky was knowledgeable and patient, taking the time to explain everything clearly, demonstrate each exercise and guide me through a full workout to ensure I understood the correct techniques. This approach was particularly helpful. The premises are very clean and well-maintained with several consultation rooms and a spacious rehabilitation and exercise area equipped with all the necessary tools and equipment. The reception staff are equally impressive - polite, friendly and always willing to help or answer any questions. Form Recovery also offers a range of other therapies including acupuncture, acupressure, lymphatic drainage and traditional Thai and Chinese treatments. Communication is excellent too with prompt responses whenever you have questions. All in all, this was a great experience. Professional care, a welcoming environment and genuinely attentive service. I will definitely be returning. Thank goodness for places like Form Recovery!Posted on Google![]()
